What is Total Iron?
Total serum iron measures circulating iron bound to transferrin in plasma — the immediately available iron pool for erythropoiesis. It is a short-term marker of iron trafficking, strongly influenced by fasting, recent intake, inflammation/hepcidin signaling, and time of day rather than iron stores alone.
